• All memberships include an orientation session prior to any unsupervised use of the fitness equipment. Members must follow directions from staff. All exercise including use of the weights and machinery designed for exercising shall be at the member’s sole risk.

• Your membership may not be assigned to or used by another person.

​

• Any equipment or property damaged /destroyed at Schaal Physical Therapy and Fitness Center, LLC, due to negligence or willful misuse by the member will be repaired or replaced at the member’s expense and may result in a loss of membership privileges at our discretion.

​

• Schaal Physical Therapy and Fitness Center, LLC, is NOT responsible for any accidents that may occur with exercise and weight equipment on the premises during any staffed or unstaffed hours.

• Schaal Physical Therapy and Fitness Center, LLC, is not responsible for lost or stolen items.

​

• Each member must use his or her own after-hours access card when entering the facility.

​

• Schaal Physical Therapy and Fitness Center, LLC, prohibits anyone from entering the facility during unstaffed hours that is not an after-hours access member. No guests are permitted at that time. Permitting a guest to enter will automatically result in a $75.00 charge to your account and possible loss of membership privileges. 24-hour surveillance is in use.

• Persons under the age of 16 may not use the facility during unstaffed hours without being accompanied by an adult member. *Absolutely NO children under the age of 13 are permitted in the gym unless prior arrangements have been made*

Applicant’s Statements

 

The signee acknowledges that he/she has been advised to receive a physical examination and consult with a physician before joining Schaal Physical Therapy and Fitness Center, LLC. The signee certifies that he/she is in good health and has no conditions or limitations that would prevent him/her from utilizing the selected membership.

 

Cancellation and Refund Policy

This agreement may not be cancelled or refunded. If you believe your situation warrants consideration please contact our office. There will be a $30.00 service charge on all returned checks or EFT. In the event that the facility closes and ceases doing business, the signee will no longer be obligated to make payments under this agreement. The signee has read and agrees to the above terms.

 

Release of Waiver of Liability

The signee requests permission to participate in weight training, swimming, and cardiovascular training at Schaal Physical Therapy and Fitness Center, LLC. The signee acknowledges that voluntary participation involves risk of injury, will hereby assume all risk of injury that may be sustained in connection with such participation and agrees to hold Schaal Physical Therapy and Fitness Center,
LLC, its employees and agents from any and all liability and does further waiver and relinquish any and all rights the signee may have to negligence, other fault or unavoidable accident, resulting from the signee’s participation in activities through Schaal Physical Therapy and Fitness Center, LLC.

The undersigned participant hereby accepts the risk associated with participation in weight training, swimming, and cardiovascular training and certifies to be of sound medical health. The undersigned hereby agrees to all of the terms and conditions as herein set forth.

The undersigned states that they have carefully read the above and forgoing document; the undersigned knows the contents thereof have been accurately and truly given and sign the same of their own free act. If the undersigned participant is under the age of 18, they will need a parent’s signature below.
