* Please note – New membership activation and membership renewal occurs during business hours 7am-5:30pm M-F.
*All memberships include an orientation session prior to any unsupervised use of the fitness equipment. Members must follow directions from staff. All exercise including use of the weights and machinery designed for exercising shall be at the member’s sole risk.

*Your membership may not be assigned to or used by another person.

*Any equipment or property damaged /destroyed at Schaal Physical Therapy and Fitness Center, LLC, due to negligence or willful misuse by the member will be repaired or replaced at the member’s expense and may result in a loss of membership privileges at our discretion.

*Schaal Physical Therapy and Fitness Center, LLC, is NOT responsible for any accidents that may occur with exercise and weight equipment on the premises during any staffed or unstaffed hours.

*Schaal Physical Therapy and Fitness Center, LLC, is not responsible for lost or stolen items.
*Each member must use his or her own after-hours access card when entering the facility.

*Schaal Physical Therapy and Fitness Center, LLC, prohibits anyone from entering the facility during unstaffed hours that is not an after-hours access member. No guests are permitted at that time. Permitting a guest to enter will automatically result in a $75.00 charge to your account and possible loss of membership privileges. 24-hour surveillance is in use.

*Persons under the age of 16 may not use the facility during unstaffed hours without being accompanied by an adult member.
*Absolutely NO children under the age of 13 are permitted in the gym unless prior arrangements have been made.
The signee acknowledges that he/she has been advised to receive a physical examination and consult with a physician before joining Schaal Physical Therapy and Fitness Center, LLC. The signee certifies that he/she is in good health and has no conditions or limitations that would prevent him/her from utilizing the selected membership.
This agreement may be cancelled if the signee becomes permanently disabled or provides written evidence signed by a physician of this disability to Schaal Physical Therapy and Fitness Center, LLC. Should the signee permanently move his/her residence more than 60 miles from Schaal Physical Therapy and Fitness Center, LLC, this agreement may be cancelled on thirty days’ notice, except that the signee is still responsible for payments due up to the day of cancellation. Such cancellation is subject to a $25.00 cancellation fee.
There will be a $30.00 service charge on all returned checks or EFT. In the event that the facility closes and ceases doing business, the signee will no longer be obligated to make payments under this agreement. The signee has read and agrees to the above terms.
I give permission for my child to work out at Schaal Physical Therapy and Fitness Center, LLC., with another adult present because he/she is under the age of 16.
